Coordinator II – RWSP Patient Navigator (BCCS Program)
Job Summary:
To provide patient navigation services for individuals enrolled in the Texas HHSC Breast and Cervical Cancer Services (BCCS) program. Facilitates timely access to diagnostic and treatment services, reduces barriers to care, and ensures compliance with HHSC program requirements through effective care coordination, documentation, and patient support.
Job Duties:
Serve as point of contact for BCCS patients requiring diagnostic follow-up and treatment services
Coordinate care across multidisciplinary teams including primary care, radiology, oncology, and specialty providers
Track patients through screening abnormalities, diagnosis, and initiation of treatment. Document visits and results in HHSC systems.
Schedule appointments, referrals, and follow-up services.
Assess barriers to care including transportation, financial, and social determinants.
Provide patient education and connect to community resources.
Document navigation activities in Epic EMR.
Utilize Epic work queues and tracking tools.
Ensure compliance with HHSC BCCS timelines and requirements.
Assist with eligibility and Cancer Stop coverage coordination.
Support reporting, audits, and quality initiatives.
Collaborate with providers and care teams.
Participate in meetings and process improvement efforts.
Performs other duties as assigned.
Preferred Qualifications:
Clinical experience in a healthcare setting (e.g., Medical Assistant, LVN, or similar role)
Experience in scheduling and coordinating appointments
Working knowledge of electronic medical records (preferably Epic EMR)
Ability to work independently and with strong verbal and written communication skills
Strong organizational and communication skills
Experience with HHSC BCCS/Cancer Stop program
Knowledge of breast and cervical cancer workflows
Experience in patient navigation or case management
Bilingual in English and Spanish strongly preferred to better serve our diverse patient population
Attention to detail and accuracy
Ability to coordinate care across systems
Minimum Qualifications:
Associate’s degree or equivalent; 3 years related experience.
Work Schedule:
Full-time, 40 hours per week. Monday through Friday, 8am to 5pm.
Onsite office located at the Galveston campus.
Salary Range:
Actual salary commensurate with experience.
