Social Worker, LMSW - Community Health Network
Minimum Qualifications:
- Master’s degree in social work from a school of social work accredited by the Council on Social Work Education (CSWE)
Graduate-level internship may be considered as experience
LICENSES, REGISTRATIONS OR CERTIFICATIONS
- Licensed Master Social Worker (LMSW) in the State of Texas required
Job Summary:
Social Work Professional - Practice is characterized by independent decision-making and a reliable approach to holistic patient care. As a member of a patient’s care team, the LMSW coordinates and collaborates with other stakeholders both at UTMB and in the communities we serve.
Job Duties
Advocates for patient and family empowerment and independence to make
autonomous health care decisions and access needed services.
Utilizes principles of human growth and development over the life span and assists
patients in coping with loss and grief related to disability and chronic illness.
Provides consultation to treatment team members regarding the psychosocial needs of patients and the impact of identified needs on the patient’s health care planning and
adherence to treatment.
Provides care that is respectful of and responsive to individual patient preferences,
needs, and values, and ensures that patient values guide all decisions. Supports
compliance with treatment protocols and assists with resource linkage as part of a
multidisciplinary healthcare team.
Coordinates with appropriate departmental and institutional resources to help patients
manage financial barriers to care.
Demonstrates a commitment to continued learning and professional growth.
Develops and implements evidence-informed care plans that promote client well-being
and ensure a client- and family-centered continuum of care.
Applies evidence-informed practice methods to facilitate the client’s accomplishment of
goals and objectives.
Ensures care continuity through appropriate transition between different levels of care
and client follow-up.
Salary Range:
Commensurate with experience.
Work Schedule: Full-time, 40 hours per week.
