REQUIRED EDUCATION / EXPERIENCE:

Registered Nurse with a Bachelor Degree in Nursing (or related field) with current Texas

licensure.

A minimum of 3 years of surgical/medical experience.

 

WORKING ENVIRONMENT/LOCATION OF POSITION:

Standard hospital and clinic environments with occasional travel to other hospitals, clinics, or

outpatient centers. Rotating responsibility for after hour’s on-call activities.

 

JOB SUMMARY:

With minimal supervision, serves as a clinician with advanced knowledge and skills demonstrating a high degree of professional competence and autonomy in the care of patients within the transplant program.

Serves in a variety of roles including clinician; collaborator; educator; consultant; researcher; and advocate to patients, families, and staff as necessary. Additionally acts as a consultant/ liaison to internal and external customers.

The clinical transplant coordinator (CTC) is a designated member of the transplant team and is primarily responsible for coordinating clinical aspects of patient care throughout all phases of transplantation and living donation. The CTC works with patients and their families from referral, through the transplant evaluation and continuing during and after transplantation, in a compassionate and tactful manner in order to provide and facilitate access to continuity of care. The CTC role may support the ventricular assist device program as defined in the essential job functions below.

 

ESSENTIAL JOB FUNCTIONS:

Pre-Transplant / Evaluation

· Demonstrates understanding and the ability to facilitate/coordinate evaluation and OPTN/UNOS listing of potential transplant candidates and living donors.

Pre-Transplant Waiting Period

· Demonstrates knowledge and the ability to monitor and coordinate care of the candidate awaiting organ transplantation. Maintains knowledge of CMS/UNOS requirements for transplant centers and ensures completion of required documentation.

Peri-Operative Period

· Exhibits knowledge and ability to facilitate the transplant process when an organ becomes available for the transplant candidate. This includes defining and adhering to the current OPTN/UNOS regulations and allocation policies.

Post Transplant In-Patient Period

· Demonstrates understanding of post-transplant management including medications, complications, interventions and discharge planning in the coordination of recipient care.

· Demonstrates the ability to collaborate effectively with the multi-disciplinary team.

Post-Transplant Out-Patient Period

· Demonstrates knowledge and the ability to coordinate the care of the transplant recipient in the outpatient setting to achieve optimal physical, social and emotional rehabilitation.

Living Donation

· Demonstrate knowledge and ability to facilitate living donation/ transplantation and utilizes that knowledge to coordinate the care of the living donor for the purpose of evaluation and live organ donation.

Ventricular Assist Device Coordinator Responsibilities

· Responsible for the coordination of care for all patients requiring implantation of a cardiac assist device including patients awaiting heart transplant or those having a device implanted as a permanent treatment option.

· Serves as a member of the cardiac transplant team and is the primary liaison for the transplant cardiologist and cardiothoracic transplant surgeons, other physicians including referring and primary physicians and hospital staff responsible for implementation of clinical interventions.

· Serves as a primary resource for internal and external customers in the care of VAD patients.

· Evaluation and education of the patient and their families pre, intra op, and post implant through discharge to the community.

· Participation in daily rounds, inpatient care and discharge planning post operatively.

· Cross train to provide coverage for heart transplantation and advanced heart failure patients.

· Provision of on call patient coverage.

· Develops, revises and presents education to VAD/transplant staff, hospital staff, patients, families and other healthcare providers.

· Participates in activities designed to promote The Texas Transplant Center and Heart Failure programs, internally and externally, to include participation in outreach events.

· Participates in the development, monitoring, reporting and analysis of performance indicators to promote patient safely, performance and quality improvement including collection of data for INTERMACS and other research activities.

Professional Development

· Demonstrates proficiency and continual growth in knowledge of transplantation and organ donation. Professional Practice

· Demonstrates understanding and the ability to assume responsibility for individual judgment to achieve the highest quality of care for the transplant patient, living donor and their family.

· Adheres to internal controls and reporting structure.
